Make a FOIL request

As part of the ongoing response to the COVID-19 pandemic, some or all agency employees may be working off-site. As such, there may be delays in response to FOIL requests.

If a record responsive to a request is only available in hard-copy format, that record’s availability will be limited until further notice. Thank you for your patience during this extraordinary time.

Submit a FOIL request online

The best way to submit your FOIL request is through our online portal. Please submit your request only once. We have centralized our FOIL processing and requests are handled by a single team. Do not use this portal for requesting copies of a police report. We have made a separate portal for that.

Request a copy of a police report

Request an MTA Police Report Online

We have launched a specific online portal for requesting a copy of a police report. This is no longer part of the FOIL process. Please use the new portal rather than the FOIL request portal.

If you'd like to mail in your report request, download the below form and fill it out. The form contains the address to mail your request to. 

Request an NYPD Police Report

We can only provide reports created by the MTA Police Department. You can find out more about requesting an New York City Police Department police report at their website.
